For adults with intellectual disabilities, the choice between institutional care and community living should never be a question of access. OHI's Home and Community-Based Waiver (HCBS) Services exist to make that choice real — providing the individualized, Medicaid-funded supports that allow people to live safely, meaningfully, and with as much independence as possible in their own communities.

HCBS Waiver programs are specifically designed for people who prefer long-term care services and supports in their home or community rather than in an institutional or group home setting—and OHI has been delivering these services in the greater Bangor area for decades. Every service we provide is built around a person-centered support plan, tailored entirely to the individual's needs, strengths, and goals.

24-Hour Residential Support Program

For individuals who need consistent, around-the-clock support, OHI's 24-Hour Residential Program provides a safe, stable, and home-like environment where comfort and individual growth are equally valued. Our staff is present every day—not just to assist with daily living, but to build genuine relationships and help each person move toward greater independence over time.

Support in our residential program includes:

  • Assistance with daily living skills: hygiene, cooking, medication administration, and maintaining a healthy home environment
  • Financial management, budgeting, and accountability
  • Community integration—employment exploration, education, volunteerism, and use of public transportation
  • Social skills development and relationship building
  • A structured, supportive environment that encourages positive life choices

For some individuals, our residential program is a transitional step—a supported home base on the journey toward independent living. For others, it becomes a long-term home, with our team providing the continuity of care that allows people to age in place with dignity, including support through illness and end-of-life care. Whatever the path, OHI walks it alongside each person.

Individualized Personal Support Program

For individuals who need flexible, personalized support—but not around-the-clock residential care—our Individualized Personal Support Program offers customized services designed to meet each person where they are. Support levels and focus areas are tailored to each individual, helping them maintain independence, engage in their community, and build on their own strengths and goals.

Lead On Program

OHI's LEAD ON! The Community Support Program is built on a simple yet powerful belief: every person with a disability has something meaningful to contribute. LEAD ON exists to help individuals discover what that is — and then go out and do it.

This is not a program that happens in a classroom or a center. LEAD ON takes place entirely in the community, provided primarily on a 1:1 basis, because real-life skills are best built in real-life settings. Every participant receives a customized plan—one that starts with their own interests, talents, and goals—and grows alongside them as they explore, engage, and lead.

What LEAD ON Members Do:

  • Explore & Volunteer—Want to give back? LEAD ON helps members find volunteer opportunities that match their passions — from caring for animals and supporting children and seniors, to helping distribute food at local food pantries. Volunteering builds skills, deepens community ties, and gives people a meaningful role in the places they call home.

  • Pursue Employment—For members interested in gaining employment, LEAD ON provides hands-on support every step of the way — exploring interests, identifying existing and needed skills, practicing the interview process, observing jobs of interest firsthand, and linking up with employment services when ready.

  • Build Independence—LEAD ON supports members in developing the practical skills that make independent community life possible — including using public transportation, managing money and finances, and communicating more effectively with others.

  • Develop Relationships & Social Roles—One of the most important things LEAD ON does is help members build genuine relationships and circles of support in their communities — friendships, connections, and belonging that go far beyond formal services.
