For adults with intellectual disabilities, getting there often requires a knowledgeable, dedicated partner — someone who understands the system, advocates fiercely, and helps turn goals into reality.

That's exactly what OHI's Community Case Management team does.

OHI is approved by the Maine Department of Health and Human Services to provide community case management services for adults with intellectual disabilities across six counties in Maine: Penobscot, Piscataquis, Somerset, Hancock, Washington, and Waldo. For nearly 50 years, we have been one of the most trusted providers of person-centered IDD support in the state — and our dedicated case management team participates in bi-monthly clinical consultations to collaboratively review complex cases and navigate ethical dilemmas, ensuring the highest standards of care.

Our Approach: Person-Centered from the Start

No two people have the same goals, needs, or vision for their lives. That's why our case managers begin with a thorough assessment of each individual's strengths, needs, and aspirations. From there, we work collaboratively with the individual — and with their family, service providers, and community partners — to develop a personalized Person-Centered Plan that reflects what matters most to them.

Our case managers then stay engaged over time: linking individuals to services, advocating on their behalf, monitoring progress, and adjusting plans as life evolves.
