Grounded in integrated behavioral health principles, our Behavioral Health Home model brings together physical and mental health care through a coordinated, team-based approach. We support adults living with mental health challenges who are also seeking help to improve physical health, manage care, and build strong community connections. Whether individuals are living alone, with family, in group homes, or experiencing homelessness, our team meets them where they are.

Think of it as a dedicated support system that surrounds you: a committed team that coordinates everything, advocates for your needs, and ensures that no matter how complex your situation, you are never navigating it alone.


Your Dedicated Care Team
At the heart of our model is a multi-disciplinary team built around you:
- A Team Coordinator who serves as the administrative/supervisory role, working with the rest of the team to ensure you are getting the most out of the program
- A Behavioral Health Coordinator who serves as your primary case manager—your consistent point of contact and advocate across all services
- A Certified Intentional Peer Support Specialist (CIPS) who draws on lived experience to help you build natural supports, peer connections, and a stronger sense of belonging in your community
- A Clinical Coordinator oversees the coordination of client care and ensures clinical and regulatory compliance across a home-based treatment team
- A Nurse Care Manager who monitors your physical health, bridges the gap between medical and behavioral care, and ensures your whole health picture stays in focus.
This team works in close collaboration with your primary care providers, psychiatric providers, and any other specialists involved in your care—creating the seamless communication and coordination that fragmented systems so often fail to deliver.
